Anesthesiology

Includes physician, doctor, or provider's time to complete a visit or procedure that includes evaluation and management of your care.

Doctor

A brace or support is frequently needed with this surgery. You may receive a bill for these supplies.

Does Not Include Orthopedic Braces and Supports

Your doctor may determine the need for medical clearance which may include diagnostic testing prior to surgery. In this case, you may receive a separate charges for pre-op clearance.

Does Not Include Pre-Operative Clearance

Includes supplies, equipment, devices needed during treatment, and service being rendered.

Post-operative visit(s) related to the patient's recovery from the procedure.

First Post-Operative Visit

Hospitalist

Service with provider either day before or day of surgery in which patient is evaluated, informed, and prepared for surgery.

Pre-Operative Visit

May include up to three (3) days as may be medically necessary.



Overnight Stay

<ul><li>Office Visit or Consult with Physician may be required to determine the need for surgery.</li><li>You may be required to bring copies of most recent images.</li></ul>

Can include any imaging procedure either before, during, or after based on physician's evaluation and need for imaging.
