An occupational therapy initial evaluation is needed for an occupational therapist (OT) to gather accurate and useful information to identify the needs and problems of the client to plan intervention and a treatment plan. A care plan is determined during the initial evaluation that is put into action during subsequent follow-up visit(s). Occupational therapy is a form of therapy for those recuperating from physical or mental illness that encourages rehabilitation through the performance of activities required in daily life. The techniques used during the evaluation process can be classified into three basic procedures: (1) interview, (2) skilled observation, and (3) formal evaluation procedures.
